Why 4-H Forms Matter
In 4-H, we believe in learning by doing—and doing it responsibly. From club enrollment to livestock projects and youth safety, our forms and documents help ensure a smooth, organized, and safe experience for everyone involved in the program.
Whether you're a parent, volunteer, or youth member, staying on top of the required paperwork is essential. These documents are more than just red tape—they’re tools that help us:
- Ensure youth safety and well-being,
- Maintain accurate records of participation,
- Comply with county, state, and national 4-H policies,
- Track achievements, awards, and project progress,
- Keep communication clear between families, leaders, and staff.
Looking for Forms?
Use the menu on the left to find all current Officer Forms, Community Club Leader Forms, and Treasurer Forms. Each section contains the documents, templates, and resources you need to support your role in 4-H.
- Annual Project Plans are now being asked for ALL 4-H Projects.
Why are we introducing this? We want to make sure that both our members AND our volunteers have a good experience. The Annual Project Plan protects Project Leaders by establishing expectations up front, and it allows us to ensure the following:
- Each project has at least two approved leaders.
- Members are only placed in projects with confirmed leaders.
- Project activities are well organized and taking place.
